Default creating a form in Word

I'm wondering how to create a form in Word that pops up when you open a
template. then you fill in the form and "submit" and those fields
automatically fill in the template. I've used forms like these but don't
know how to put one together and I'd like to find out.

thanks!

G'day Abi ,

Check out what the word.mvps.org FAQs have to say on UserForms. The
help is quite extensive.
