Default how can i let users insert a picture in a form

I want to distribute a form that user can click on a button, which will
display an "Insert File" dialog box. From this they can select a picture and
click OK, and then this will be embedded in the document.

1) Is this possible and;
2) I'm totally new to this 'form business' and have no knowledge of visual
basic and;
3) CAN ANYONE HELP!!!!
