How do I create a "template" in Word where prompts pop up when opening the
template to enter information to be placed in various places in the document?

I would recommend using either a UserForm
(http://word.mvps.org/FAQs/Userforms/CreateAUserForm.htm) or what Word calls
an "online form." Check this in help. For more about online forms, follow
