Default Chapters and Subtitles in TOC

I have chapter numbers with sutitles. For example, centered at the top of
the page:
CHAPTER 1
DAILY OPERATIONS

When I insert the TOC, I get:

Chapter 1..............1
Daily Operations.....1

How do I get "Daily Operations" to be on the same line as "Chapter 1?"

Use a line break instead of a paragraph break at the end of "Chapter 1," and
the entire paragraph will be included as a single entry. The line break will
be interpreted as a space. If you want punctuation at the end of "Chapter 1"
in the TOC (or if, as is more usual, you want just the number), use
different styles for the chapter number and chapter title and include only
the title in the TOC. Add the numbering (and label, as desired) to the TOC
style instead.
