Default In a form, add "Additional" yes/no box if yes, dropdown & fill-in

In a Word form, I have placed a text box that would allow teachers to add the
name of a book for their class. If the teacher had two or more books for the
same class, I want them to be able to choose a Yes/No box and then another
dropdown "area" would show and they could fill-in the text boxes in that
dropdown.

How do I do this in a Word 2003 form?
