Default Caption and TOF issues

I am working in Word 2000, and I am having a lot of problems with field codes
skipping numbers and with the table of figures as well:

Skipping Numbers:

The caption field code is: SEQ Figure \* ARABIC \s 1

What we find is that we have figure, say A-8, and then the next figure in
the series is Figure A-13. There are no instances of the caption style in
between the 2 captions, no other text, no pictures, no breaks, only 3 Hard
Returns. If I try to enter a caption through Insert--Caption..., it shows
Figure's A-8 and then A-13.

TOF:

In that same blank area as above I tried to insert a picture into the 2nd of
the 3 hard returns. The picture then appeared in the TOF as a new figure,
with no caption. I did not have to add a caption at all, and the actual
picture is appearing in the TOF.

The TOC field code is: TOC \h \z \c "Figure"
The Individual entry code is: HYPERLINK \l "_Toc96939171"

Any help on these 2 problems would be greatly appreciated. Thanks.

The empty paragraphs into which you're inserting figures are not in the
Caption style? I would expect them to be if they were created by pressing
Enter at the end of a caption. I think you'll find if you change the style
to, say, Normal or Body Text or (best of all) a custom Figure or Graphic
style, both your problems will be solved.

The 3 Hard returns were set in normal mode. I selected everything from the
caption above the picture (the caption for the previous entry) all the way to
the caption for the next picture. I then changed everything to normal, even
the 2 caption entries, and still the actual picture appears in the Table of
Figures, even though it's all normal text. Also I tried deleting the
individual Field Code entry for that figure, and word repopulated the field
code when I did an update.

Have you updated the TOF (using F9) since making these changes?

I have.

I have even selected everything inbetween the caption that reads A-8 and the
caption that reads A-13, to the point that both captions were on the same
line and running together and still caption A-13 would not change to A-9
(after trying to update the field code.) This document has been cut and
pasted into a lot so I'm thinking there is a corruption in the numbering. I
even tried deleting and inserting a new TOF and the same errors showed.
