Why do some hyperlinks return invalid site locations?

I have seen this with hyperlinks that have the correct URL as the display
text but an invalid one in the actual field code (this can happen when you
have modified an existing hyperlink). If an incorrect URL is displayed in
the ScreenTip, then the field code has not been updated, and the link won't
work. In some cases, if I select the entire link, the Edit Hyperlink dialog
shows everything correct, but if I just click in the link and go to Edit
Hyperlink, I see that the URL is still incorrect.
