Cannot add icon to toolbar; could not drag the command

You could try resetting the toolbars -- Tools Customize Toolbars tab, Select Standard and press Reset, and then select Formatting and press Reset. If that doesn't help, I wonder if you have some corruption. You could try Detect and Repair.
