cross referencing with track changes and versioning

any idea why a document on a third version would lose all cross referencing
which was inserted on a second version? Amendments have been tracked changed
and its the tracking which I suspect may be causing this. Also this occurred
when document sent to print - didn't manually request fields to be updated.
'Error bookmark source' code instead of cross ref.
