Presumably this refers to Word 2007 -- something you should always include in any question here. Admittedly, the feature is hidden in one of the most improbable places Microsoft could have stuck it: When the picture is selected, go to the Picture Tools ribbon, click the little dialog launcher button in the bottom right corner of the Size group (Size??? what the #$%^ does size have to do with Alt text?? Why isn't it on the Format Picture dialog?), and click the Alt Text tab of the dialog. But... you could have discovered this by asking Google to find the terms "Word 2007" and "alt text" together, and finding http://support.microsoft.com/kb/923919.
