Default Address problems after merging with excel file

My original document uses merge fields as follows:

«First» «Last»
«Title»
«Company»
«Address1» «Address2»
«City», «ST» «Zip»
Dear «First»,
____________________________

However, after merging with excel database, it chcanges capitalize letter,
deletes merge field and replaces with another, adds address block field in
first paragraph, and looks like this:

«first» «Last»
«Title»
«Company»
«Address1» «Address1»
«City», «ST» «Zip»
Dear «first»,
_____________________________________

Any suggestions?
