Default Document Map

I have a document with the document map showing. The document map has a few
entries with the expand/collapse indicator for the entry. When selected, the
indicator only toggles between '-' and '+'.

There is no sub-entries for display, how do I hide the indicator?

I have other entries that don't have sub-entries, and the indicator is not
displayed for them.

Hi John,

You have empty paragraphs in a heading style?
Empty paragraphs are best avoided, not only because they appear in =
Document Map.

To find them, use ^p^p in "Find what"... and maybe ^p^w^p too, for =
paragraphs that only contain whitespace.
