Default Labels from text field in a form

I think this problem may require a little VBA which I've done before in Excel but not word.

Here's my problem. I've got a letter of transmittal I created as a template form. It's got various fields including a mailing address field that is simply a text field to be manually entered. What I want to do is create a macro or somthing similar that will print a label based on the text in this field.

I also have a company logo that I would like to print as the return address.

In addition I would like to distribute this form to various people that would need to be able to use the same macro so it needs to be attached to the form.

Like I said I have a little background in VBA if required.
