I am trying to set up a text box for phone surveys and it lists the phone
number of the person called. I would like the form to format the phone
number into ###-###-#### form but I get a invalid number error.

If this is a form field, set the field as a number and enter the mask
exactly as you have it in the number layout box.
Don't use text boxes, use frames or table cells.
