__ Court Judge
Room Number of Chambers
Courthouse address

{Subject matter}

Salutation: "Your Honor:"

Present your problem succinctly.

Request if he is permitted to assist you with the type of problem you
are presenting to him and, if so, ask him for his assistance. If not, ask
him if he has any suggestions where you might obtain that information.

Thank him graciously (copiously, ubiquitously, etc.) for any
assistance he might deem to provide you. {Short and sweet is the only way
you are likely to get any help from a judge.}
