Default Locking a document

Hi,

I have a Dear ......, letter that I need to lock, but I need to be able to
have folks fill in the Dr.'s name without being able to change anything else
on the page. How do I do this. I can only find ways to save master
documents or forms...this is just a regular letter.

You can do this with a protected form. See
