Default Form Creation

How can I insert lines (after name, address) so if someone were to open it
,they could type in the info and retain the formatting, or am I missing
something very simple?

I would probably use a table with custom borders. See:

A table without borders is good.

If you want to have [Click and type] fields you could use macrobutton
fields. See http://www.addbalance.com/usersguide...tm#MacroButton and
http://www.mvps.org/word/FAQs/TblsFl...acroButton.htm for more
about macrobutton fields.

If you want something where they can only type in places you designate, what
you are talking about is what Word calls an "online form." Check this in
help. For more about online forms, follow the links at

Chapman's series of articles. You may also want to look at
