Store this macro in the Normal.dot (for Word 2003 and earlier) or
Normal.dotm (for Word 2007) template:

Sub AutoOpen()
ActiveWindow.Caption = ActiveDocument.FullName
End Sub

See http://www.gmayor.com/installing_macro.htm.
