Default Bullets & Numbering Mess

There are documents that are created in my company that have several levels
in the outline. Example:

A.
1.
2.
B.
1.
2.
a.
b.
1)
2)
C.

Since there are so many levels, it's easy for the numbers and/or letters to
"fall" out of sequence. At times I see some numbers and letters missing from
the outline. Sometimes people duplicate numbers in certain levels, etc. How
can this be avoided? I really don't use styles at all. Can this be solved
using styles? If not, how can I end this mess?
