Default How do I fill in the blanks of a form using a pop-up dialog box?

I am developing a fill-in form. Instead of using the "tab over" method to
fill in the blanks, I'd like to use a pop-up dialog or input box to type the
information and let Word fill in the blank for me. I would like a new box to
pop up for each fill-in item. Any suggestions on how to automate the fill-in
process this way? (This is for Word version 97 and up.)
