Default How to place the index after the endnotes.

I cannot get Word 2000 to place my index after the endnotes. I have tried it
in both normal and print layout views. The Insert Index screen pops up, but
the "OK" button remains dimmed. There is probably a very simple answer to a
stupid problem. Please help! Thanks.

Default How to place the index after the endnotes.

When I set endnotes "end of section" in Word 2003 my endnotes go away. I was
able to do this in Word 2000 and get my index after the endnotes. I haven't
tried the macro because it appears to be designed for Word 2000.
