Default Word Template

I am setting up some templates in Word using "Ask" fields and Macros. I can
get the Ask field to pop up with the question and am ok saving the macros. I
am trying to find a way to create an option box in the "ask" field. In other
words, the user will see a pop up box that may say : Mr/Mrs/Dr. and they are
to click next to the correct salutation. There will be a field to complete
for the name once they do this. When the proces is complete, the letter will
be filled in with the correct salutation and last name. Thanks. this site is
always very helpful

You will need a userform to do this, rather than ASK fields - Word MVP FAQ -

A simpler option may be to use Macrobutton fields which make it more obvious
