Default Adobe PDF - legacy tools form fields not working properlt

I have created a 2007 Office Word Document that has 121 legacy tools form
fields imbedded within. Using Adobe Professional 8.1.2 I save the document
with PDFMAKER some fields are not imported or do not display in the PDF as
expected. These forms will be used by a Boy Scout Troop so any assistance
will be greatly appreciated.

Check Boxes €“ are used throughout the form (yes/no to the question). The
check box imports okay, but when I select them, when completing the PDF, they
are blackened out and the checkmark is not visible, just the blackened box.

Text Fields €“ I opted to use text fields to enter dates. These do not
import at all. The format is text/text/text. The fields are set to numeric
and limited to two character for each field.

Ive been in contact with both the Microsoft and Adobe Helpless Desks and
each points the finger to the other software.
