Default styles not applying correctly

I'm tearing my hair out. I oenened a doc today I had been working on
yesterday. I'm using an outline for headers and subheaders. I made no
changes to the styles or anything; it's a standard template I use all the
time. So I add my remaining three sections, update my table of contents, and
suddenly the text appears without the outline number, but the page number is
there. The next line of the TOC contains the outline number and page number
together. I've done the style compare and there are no differences between
the sections I did yesterday (which look great in the TOC) and the ones I
added today. Anyone ever run across this? I've used the brush to copy
styles and everything.
