Default TOC custom styled headings not remaining in template

I've created a document with 4 custom heading styles. I checked the "Add to
Template" checkbox in the New Style for each. I insert a TOC and for awhile,
they seem to work fine. But then for some reason, it reverts and only
includes the default Heading 1, Heading 2, etc. in the TOC and omits my
custom headings. When I check the Modify Style box, sure enough, the "Add to
Template" box is no longer checked. I've gone to Insert Reference Index &
Tables and go to the Options box and the styles I created are no longer
checked. I check them, UNcheck the default headings, click OK, update the TOC
and again, it works for awhile and then goes wonkers again. Any suggestions?

"Add to template" only adds the *style* to the attached template; it does
not affect the TOC settings, which will always revert to the defaults in
Word, when you re-visit the Index and Tables dialog box.

What you can do is select the existing TOC and create an AutoText based on
it. Then insert the AutoText whenever you want this particular type of TOC.
