Default Forms from a PDF or scan

I have a bunch of forms that I want to digitalize.
I will want to have the forms add some of the data through data in a
database, then get the user to add some data of their own and then read that
in and insert to the database. Tried the adobe pdf route, didn't work unless
we got all our users to get adobe pro which wasn't going to happen.

Anyways I have these forms that I scan in or add from a pdf, or export from
Cyrstal Reports, and unfportunately it is not looking very easy to do.
Getting the formatting done seems to be the problem as it does not look like
it is possible to just drop a form field wherever you want it.
ps the reading in and all that stuff I can do, don't need help on that, just
the making of the forms themselves)
If anyone has any suggestions or help or solutions that would be much
appreciated.

Default Forms from a PDF or scan

If you anchor the image of the form in the header of the document, it will
appear as a watermark and you can then insert a table in the bodyof the
document to position formfields in the locations where you need the users to
add their data.
