I have created a form template and want to preserve the blank form. How do I open the doc from one folder, complete the form and then automaticlly save it to another folder when click save.

Make sure to save the form as a template (*.dotx) and place it in the user
templates folder on your system. That way, you will get an untitled document
each time you create a new form, and the original file (the template) will
be left intact.
