Default Shift Key

The Shift key does not always give upper case on first press. If held down,
second press produces the capital letter. What have I done to the formatting
and how do I undo it?

Probably you have (perhaps inadvertently) enabled some of the accessibility
options in Windows; see Control Panel | Accessibility Options.
