How do I create my own template document (e.g., a referral form)
I would like to create a template for a referral form. (so others can fill in
the blanks electronically). How do I do this on Microsoft word? |

I expect that what you are asking about is not a Word template
(http://addbalance.com/usersguide/templates.htm) but rather what Word calls an "online form." Check this in help.
