Default track changes reviewer identity

Is it possible for track changes to mis-identify who has made what insertions
or changes? If people are using different versions of word could that
happen? When I cursor over an insertion (in my home, on my computer) the
author is identified as someone other than the person who claims to have made
the changes (in his home, on his computer). Is that possible? Any help is
really appreciated!
