form creation help
I need to create a simple form that will have radio buttons, drop down lists,
and check boxes, the difficult part is that it needs to be used in my ACT database and would need to be converted to an HTML template that can be used to complete a survey form and then email off out of the database. Does micrsoft word have the capabilities of creating this type of form and then converting over to html? |

You would be better spending your time in FrontPage developing the form for
this purpose.
