Default create form with lines

I'm trying to create a form for employees to fill out when sent to them
through outlook mail. the issue is this, if i use a text form, the lines move
underneath and if i use tables, when i hit the tab button to go to the next
form it just adds another line underneath the existing one. what am i doing
wrong? how can i get a form that goes from question to question? Thanks a lot!

What you are talking about is what Word calls an "online form." Check this
in help. For more about online forms, follow the links at
