Default How do I insert a multiple choice, clickable button

I am desiging a post-training survey. I want to send it electronically afte
the course. The format is multi-choice e.g. "did you find session 1 useless,
quite useless, okay, useful, very useful" I can say "tye number in here" but
it would be more elegant to have the attendee click on a button, that turns
the button black and, when sent back to me, I can see it is a vote.
