Insert a three-column table. Drag the column widths to where you want them.
If you don't want the borders to print, press Ctrl+Alt+U to turn them off.

Apply the List Number style to the leftmost cell.

In the center cell, press Ctrl+J (or click the Justify toolbar button) to
make that cell justified.

In the rightmost cell, insert a decimal tab stop (see
http://www.word.mvps.org/FAQs/Format...ettingTabs.htm) at the location
where the decimal points should align. Because the text is in a table, you
don't have to press the Tab key when you enter numbers there; they'll
automatically align to the decimal tab stop.

Each time you need a new entry, put the cursor at the end of the rightmost
cell in the last row and press the Tab key. All the formatting will be
copied into the new row automatically, and the leftmost cell will show the
next number; just tab again to get to the middle cell and start typing.
