How do I insert a box that will allow the reader to place a X in .
I am creating a Survey using word and I want to insert boxes so that the
reader can place a "X" or a check in it. I would appreciate any assistance. |

I have insert the check boxes using the Forms options. Next I want to know
how allow the reader to only choose one box per statement. For instance, for one statement there are 5 choices but I want to only allow to select one or the other.
