I need to request a hearing in criminal court to modify my sentence from
community control to regular probation.

heart of your problem.

Default writing a to a judge

I have no idea what community control is (nor do I want to find out) but
this newsgroup does not help with the content of Word documents, only the
format and how to use Word.
