Is there a form field or can you insert a numbered list into a form field in
a protected document?

Default Insert Numbered List in Form Field

You can apply a numbered list to the text form field *before* protecting the
document.

Beyond that, you can add a section to the document and leave it unprotected.
Users will then be given more options to format text. Note that in Word
2007, they can only make use of Quick Styles (the ones that you see in the
Styles group on the Home tab).
