Default making simple forms like lists

I want to be able to make a simple to do list for my grandson for his various
activities and household chores. I don't want to draw the check boxes myself
(I am too lazy for that), I want one that is already made. looked at
templates on M/S office online but didnt see anything. and $99 for one note
doesnt get it for me as i probably wouldnt use any of the other features.
