Default Creating Forms

I would like to create a form that is a fill in the blank type form. One
that has standard information that never changes with fields that you can tab
to and fill in other information. Does anyone know how I do this?

What you are talking about is what Word calls an "online form." Check this
in help. For more about online forms, follow the links at
