I have a running header that I have inserted by using INSERT-- FIELD --
StylRef. The header shows up on several pages and then doesn't appear on one
of the pages. If you highlight the area and "toggle field", the formating is
there but the words do not appear when you toggle back to regular view. Any
idea what might be going on?

It sounds like the STYLEREF is picking up an empty paragraph or perhaps a
page break in the referenced style. Use Find to search for all paragraphs in
that style (open up the dialog with More and select Style from the Format
button menu in the dialog)
