Default Can't print all records

I am trying to print records of attendance certificate on a template Avery
Index card 05388. The data source is from an Access query. It merges only
the first 15 records of 21. Not sure why. Look everywhere and can't find a
solution. also, the formatting is lost when the merge occurs in a new word
document.

Thanks for the help.
