I have a a situation where when you hover over a change in a tracked
document, it flashes the comments box for only a brief moment and then
disappears. I have rebuilt the Normal.dot, I have run an Office Repair
through the Add/Remove Programs, I've even adjusted the Hardware Accelerator
in the Display options of Windows XP -- nothing has helped yet.

Any ideas of the cause and/or fix?

There was an error in Word 2002 where this behavior was noticed with
footnote/endnote ScreenTips. The solution there was to turn the document on
and back off. Worth a try, anyway.
