Default Goof-up with renumbering

I have a document in which I have first, second, and third level headers
(i.e., 1.0, 2.0, 3.0). The list numbering that follows 1.0 works fine.
1.
2.
3.
In the list following 2.0, I have to right-click "Restart Numbering". That
works, and the list is fine.
1.
2.
3.

After 3.0, I right-click the first list item to restart numbering and I get
this:
4.
1.
2.

If possible, please help me understand what's going on and how I can make
numbering work correctly. Your help will be GREATLY appreciated.
