Default Indexing -

instead of group similar index entries together some but not all are listed
separately even though the entries are exactly the same. Why?

Are you sure they are identical?

Entry{ XE "Entry" }
Entry{ XE "Entry "}

May look identical at first glance, but notice the extra space after the
last 'Entry'. This would force it to show up as a separate entry in the index.
So, first, I would Show the nonprinting characters and look at the offending
index entries to make sure they are exactly the same.
