Default Fill-In Forms

I'm also very frustrated in attempting to create a simple "Fill-in" form in
which I can "protect" to allow the user to tab between the fill-in fields and
enter data. I'm not a novice nor am I obtuse, but I can't seem to figure
this out. Does anyone know of any written guidance to follow this out?
