Default Create document that prompts for fill-in fields

This would be a boiler plate document that would prompt for addresses,
phrases, etc.

If you already have the fill-in fields in the document then save the
document as a template. When you create a new file based on the
template the fill-in fields will automatically display.

assistance by email can not be acknowledged.
