Default Checkbox in Word 2007

I'm using Word 2007 and I need to insert a check box that someone can check
electronically in the document. In previous versions of Word I would open
the Form toolbar and insert a checkbox. But I can not find the Form toolbar
in the new version.

Default Checkbox in Word 2007

Courtesy of Patrick Schmid (posted earlier today in this newsgroup):

Office button, Word Options, Personalize, activate Show Developer tab in
the Ribbon. Click OK.
Now you can find the Forms control on the Developer Tab, Controls group.
