Default Automated Revision History Table

I have seem them in a million documents, tables at the beginning or end of a
document that show the vrevision history of a document - Last Edited By,
date, Comments - that sort of thing.

Can you please shed som light on how this can be automated - that is - in
the document where I have Versioning turned on (Word 2003) I want to create a
table that will on each successive new version - or edit - create a new row
in the table and then populate it with the relevant version info (name date
etc.)
