Default Indexes and page numbering

Im using word 2003 and Im trying to create an index that looks like this

Plants and Animals..............45 - 75
Plants............................45 - 55
Oak Tree..........................45
Pansy...............................47
Animals.........................56 - 75
Cat..................................56
Dog..................................57

I can get the lowest levels eg cat, dog, oak tree, pansy to have page
numbers but I cant get Animals, Plants or 'Plants and Animals' to show page
numbers. There doesnt seem to be a format option that does and I cant figure
out any field switch that I can add that will do it.

Any help greatly appreciated
