Default Disappearing Data on Returned Form

Using e-mail, I am sending out a "protected" MS (Word 2000) form to
clients that I created. They are asked to save it to their hard drive, open
it, complete the form, save it, then attach it to an e-mail back to me.
The returned form is showing completed on the e-mail preview screen but
when I save and then open the file, the form apparently resets, and the data
is erased.
Thanks in advance for your any advise!

Any chance you sent out a template (.dot) file and are creating a new
document from it when it is returned? What is the name of the document shown
in the title bar?

I rechecked and both outgoing and incoming files were .doc types.
